If you are a new to Perkins Road Pet Clinic please also fill out the Client Information form and Additional Pet Information form if needed. Both of these are found under Forms on this website.

Thank you for choosing to board with us. In order to maintain a healthy and parasite free boarding facility all pets must be current on the following tests and vaccines. If your pet is not current, these services will be provided upon admission to the kennel AT THE OWNERS EXPENSE. PROOF OF VACCINATIONS IS REQUIRED.

Has your pet had the following tests and vaccines:

*Your pet will be treated on entry for fleas while boarding at the cost of $6.36 per pet.

While we are caring for your pet the following information will be helpful...

There is a $4.83 charge for each AM and PM treatment for medications.

Pet's have to be at the clinic by 6pm Mon-Fri, and 4pm on the weekends. This allows us time for check-in and to perform any services needed.

Please leave a contact number where you, or someone authorized to make decisions regarding your pet, can be reached.

We reserve the right to treat your pet for any medical problem we deem serious, at the owner's expense. Attempts to contact you will be made at your emergency number.
